This fee covers 12 months of qualifying for the master specialist designation. Once you submit this form with your payment, Lisa Sheldon will follow up with you to discuss the next steps in completing the master specialist certification. There are no current requirements you must satisfy in future years to maintain this certification - but, you must remain a DWHSA member in good standing to use the certification. By submitting this form and paying the fee, you agree that you understand this rule and you acknowledge that you must cease using the designation and the logo if you decide in the future not to renew your DWHSA membership.
